New Patient Registration Assistant
The registration process takes 15 to 20 minutes and will provide you with all of the necessary steps to get set up to participate in care with our practice. At any point during the registration process you encounter concerns or have questions please reach out to us by phone. The registration “assistant” below will take you through the process in a stepwise fashion to make the process as streamlined as possible.
You only need to complete this process once time and after the process is complete and you will have access to our services in as few as 3 clicks of the mouse throught the “Choose An Appointment” button on the top of our home page. You may also always use the Spruce App to send a message, request or phone call. Thank you for choosing the Modern Country Doctor, LLC for your care. Step 1: Background
General Info
Please take a moment if you haven’t already to browse our services page. You will find a thorough list of the services we offer and an indication of the thigs we are working on. You may also want to review our clinic policies and FAQ page for additional information.
If you know what you are interested in or have had a conversation with our staff over the phone you can proceed to step 2.
Registering for Dependents or Family Members
You will use the same process outlined in the following steps if you are scheduling on behalf of someone else with the following adjustments: (Each bullet point below will also under each step below to as a reminder during the process. Alternatively you may download our dependent registration PDF and review as you proceed)
- Spruce Health Account:
- You will login to Spruce Health or download the App with your name and account, please identify information specific to the person you are communicating on behalf of by including their name or initials with each message.
- If you already have an account and are setup with Spruce you can move to the Elation Health step below.
- If you do not have a Spruce Account or App, Please follow this link: Spruce Health Download or Registration and register using your name.
- Visit Selection:
- The visit selection link on our website is integrated directly with our electronic medical record at Elation health
- You will need to adjust the process as follows:
- Please choose a “Dependent Visit” from the appointment types (Either new or established).
- In the Name and Date of Birth Fields please enter the dependent or family members information.
- In then email and phone number fields please enter the information you used to setup your Spruce Health and Elation Passport accounts (see brief graphic below).

- After you schedule the visit you will receive an additional “Dependent Visit” intake form via email that must be completed prior to the visit. Although some of the information entered here is a duplicate step it calarifies the infomation we need and allows us to ensure that we can link the proper information to the proper records.
- The “Dependent Visit” form and the telemedicine consent must be completed prior to the visit.
- Please call us prior to the visit if you are unable to complete the forms or are having trouble with the process in any way.
- The visit will be delayed or possibly cancelled if the forms are not complete OR we have not discussed the visit with you directly.
Dependent Passport Registration
- After you have completed the registration and appointment setup during your next login to Elation Patient Passport you will be given the opportunity to link your family members chart to your own.
- Elation health terminology for this process is to “claim” the chart.
- As long as the email and phone number match up with your previously registered adult or legal guardian chart exactly, the accounts will be linked during the “claim” process. (If you are not certain of this information please consider taking a moment to log in to Spruce to verify this information for accuracy prior to proceeding).
- After the “claim” in complete in Elation Passport the charts will remain linked in Passport and can be selected during future login attempts.

Step 2: Required Documents
Getting the documents:
We are required by law to provide you with the proper consent forms and HIPAA privacy notices. Please follow the following link to download and/or print the PDF download for the entire packet:
The above packet contains each of the three primary documents required to participate in care at The Modern Country Doctor, LLC. This packet contains each of the following documents. If you do not require the entire packet or need to reprint a portion follow the following links for each portion.
At the current time we are out-of-network for private insurance plans. In order to provide predictable service costs for both insured and uninsured and uninsured healthcare customers for the services we provide. As noted elsewhere we will provide a superbill at the conclusion of your service for possible partial reimbursement. Please review our services list. If you have any concerns or questions regarding this list please reach out to us prior to your registration or visit. Note: we are also developing additional options for medications, additional imaging and health related services to be offered in the near future.

Returning the signature pages:
The signature pages are the last three pages of the packet. You can scan or photograph them and send them back via the HIPAA compliant communication Spruce Health App prior to or at the start of your visit. PLEASE NOTE: Our general info email or any email from the moderncountrydoctor.com domain are not HIPAA compliant and should not be used to return these documents. As long as you have reviewed and understand the documents at the start of your visit we can review verbal consent and discuss ways to return them to us at a later time. You may also fax them to our HIPAA compliant fax line at (610) 320-2130. If you do fax please include your name and date of birth so we can match the documents to your record. Please retain the original copies for your records.
Step 3: Download or Sign-in with Spruce Health
We use two health software services to provide for Virtual Care in a HIPAA compliant manner Elation Electronic Health Records for your clinical information, orders and results and Spruce Health for text, email, Spruce Visits and communications. You will automatically be enrolled with Elation EHR during the visit selection process. Please use the following link or any of the links present on our website to access and sign-up or download the Spruce App as the links are specific to our practice.
NOTE: You need to be 18 years of age to register with and use the Spruce App for communication. Otherwise the communication with occur under a family account.

Step: 4 Schedule a Visit
What to Expect
Our scheduling is web based and linked directly to our Electronic Medical Record system with Elation Health. In order to register for a visit you will be taken to the Elation Health website. A new tab will appear with our practice schedule. You can choose and appropriate New Patient Telemedicine Visit. The following bullet points outline the steps you will experience during this process and the time that is typical for each.
- Choose an Available Appointment: (1 to 2 minutes)
- You will be required to put in your name, date of birth, email and cell phone.
- You will be sent a link via email to register with the Elation Passport Patient Portal
- Click the following link to open a new tab with the scheduling site: Available Appointments
- Complete the new patient intake form (5 to 10 minutes)
- Fill out the Demographics and Past Medical, Social and Family History sections as much as you are willing or able.
- The more thorough you can be in completing this information the more time it will save during your appointment.
- Check your email for the Elation Passport Patient Portal Button (1 to 2 minutes)
- You can view the included instruction video included in the email, or press the button to complete the registration.
- You will be sent a verification code via text message to the cell phone number you entered above.

Step 5: You are now ready for your first visit!
You are now registered and ready for your first visit. All of our telemedicine visits are currently being conducted via the Spruce Health interface. You can either use spruce on a desktop, laptop, Apple computer, android or iPhone. Spruce Visits can only be completed with a tablet or smart phone at this time. If you complete the Spruce Visit questionnaire you can then use a desktop or computer to complete the visit.
If you plan to use a digital stethoscope or otoscope during your visit you will need to complete the telemedicine visit on an Apple Computer, Laptop or Windows Desktop. Please see the section below on “Using Instruments or Equipment” for additional details
Telemedicine Visit with Apple Computer, Windows Desktop or Laptop
Prior to your visit time you will need to log in to the Spruce website to be ready to take the video request. Please complete the forms from the steps above and prepare any materials or questions ahead of time as it will make your visit more efficient. Telemedicine Visit with iPhone or Android Device
Visits with an iPhone or Android are much simpler. They have the advantage of portability and can integrate with the Spruce Visits, messaging and phone calls much easier. You will only need to have the Spruce Health App downloaded so you can receive push-to notifications when your visit time arrives. They are however limited in that they are not able to integrate additional resources or instruments at this time. Please keep this in mind when planning for your visit and/or travel. Once you have decided on your platform you are all set. Just have the Spruce App open or be logged in to the Spruce Website
Here is a pre-visit checklist to aid in the process:
- Review HIPAA Privacy Statement, Review and sign Financial and Privacy Acknowledgement, Review and Sign Telemedicine Consent (or call ahead with questions)
- Select a visit time that works for you.
- Make sure your Elation Passport Registration and Spruce Health Logins are working and up to date.
- Choose a quiet, private place to complete the visit.
- Log in or have the Spruce App ready ahead of your visit time.
- Have a notepad handy to take notes, contact info or basic instructions.
- Any additional equipment, blood pressure readings, blood sugar meter, prescription bottles or items you would like to discuss.

After Your Visit
Instructions for Initial Bill & Follow Up
There are several items that you will receive after your visit. The two lists below outline the types of items that will be forwarded and where you will find them.

You may also have received a prescription, lab test or radiology order. Prescriptions are typically managed by the pharmacy they were sent to. They can contact us directly with changes or substitutions as needed. If you need to have a lab test completed or radiology study performed we can send additional detailed instructions as to how the process will work to any additional preferred methods of communication you identified on your consent form.
If you have any questions, need changes or concerns call us during business hours or message us within the Spruce App. Although we cannot guarantee we will be present outside of normal business hours we frequently check those messages and address concerns as we are able.

Payment
General Information
As outlined in the HIPAA and Financial Acknowledgements you will be sent an invoice or payment request at the conclusion of your visit. Unless specified otherwise we will follow the services fees outlined in the Price Transparency form. If you have questions or concerns please address them ahead of your visit if at all possible. We are always willing to hear your suggestions or concerns. As outlined in the clinic policies we will supply your with a superbill for the visit and services we provide. Some of these services may be reimbursable by your plan. You can submit the superbill to your insurer for possible partial reimbursement.
Credit Cards and Electronic Payments
We are using Stripe to do all of our credit card processing. Requests for payment are generated and handled by Stripe exclusively and securely. Currently all of our requests are being sent through a message payment request in Spruce Health. Stripe and Spruce health have a direct API integration to maintain your privacy a security and minimize the risk of errors within transactions. You can access the message and provide secure payment through the link we send via the Spruce Health System. We are working At the current time to have a developer add a direct link to our website for ease of payment at a later time. See our main website for updates on this link
Direct Payments and Checks
At this time we cannot accept direct bank or ACH transfers as a form of Payment. We can send a detailed invoice for payment by check upon request. Directly sent invoices will also include instruction for providing a credit card number for payment processing without using the internet or App based link. We strongly prefer the method above within the Spruce App.
Follow-up Visits
We may recommend follow-up visits as part of your encounter. If we place a referral we will send appropriate information along to your specialist for review unless you specify otherwise.
Once you have gone through the registration process with us scheduling follow-up is much easier. You can visit our main page and simply click the red “Choose an Appointment” button at the top of the page to choose an available time or follow the link from the top navigation menus. Everything else will already have been setup and ready to use.
